If you're having an urgent, life-threatening emergency, please dial 911. If you have an urgent, non-life-threatening medical problem (cold, flu, minor injury) please call your PCP's office. Depending on appointment availability, you will be seen by either your PCP or the doctor-on-call.
If you have an urgent matter after hours, or on a weekend, you should call our main number at (510) 785-5000, or go to an urgent care center which is contracted with Bay Valley Medical Group. We suggest that you call in advance and check the office hours before an urgent need arises.

Internal medicine physicians have specialized in the medical (non-surgical) treatment of diseases in adults. They receive in-depth training in the body’s organ systems, e.g. heart, lungs, kidneys, etc. and may serve as primary care physicians for individuals 18 years of age and older.
Family medicine physicians have specialized in the medical treatment of diseases in both adults and children and are oriented to family dynamics and care. They have usually also been trained in performing minor office-based surgical procedures. Family medicine physicians may serve as a primary care physician for individuals of any age.
Nurse practitioners are registered nurses with advanced training and provide patient care including the diagnosis and treatment of illnesses. Many nurse practitioners have undergone graduate education and have master’s or doctorate degrees. They practice under the rules and regulations of the state in which they are licensed and they may be nationally certified in their specialty area. Besides providing clinical care, nurse practitioners might focus on health promotion, disease prevention, health education and counseling. Nurse practitioners may not be selected by patients as a PCP (primary care provider).
We prefer that patients phone our office to schedule an appointment even when there is an urgent, same-day need. Patients will be offered an earliest available time and if your own physician is not available, another physician or nurse practitioner will be available to see you.

Please arrive 20 minutes early to your first appointment in order to have adequate time to register and to complete all paperwork. Be sure to bring your health insurance card and billing information with you. Depending on your health insurance, you may be required to make a deposit on your newly established account. Please also bring along a list of medications that you are currently taking and any other pertinent medical history that you wish to discuss with your new doctor.
It is not required to have your previous medical records; however, it is sometimes helpful to have such information to have a complete history on file at Bay Valley Medical Group. This is best discussed with your physician. If you wish to have your medical records forwarded, contact our customer service department. They will provide you with a Medical Records Release

When medical records are transferred between physicians, there is generally no charge to the patient. However, if you request a copy of your records for your personal use, there may be a fee.
I am moving or switching doctors. How do I have my Bay Valley Medical Group medical records forwarded to my new doctor?
We will transfer your medical records to your new physician. Please allow two weeks for this transfer to occur. Please contact our customer service department at 510-785-5000 or info@bvmed.com. Our representatives are available Monday-Friday, 8:30 am – 5:30 pm and will be happy to assist you.

If you have a life-threatening, medical emergency, phone 911. If you have an urgent matter that is not life-threatening, please phone your doctor. Bay Valley Medical Group has an on-call doctor 24 hours per day, seven days a week. If you are seeing one of the doctors in our clinic sites, please phone the office and we will arrange for you to receive the proper advice and care. If you are seeing one of our affiliate physicians, please phone them directly.

When you need a refill on your medication, please phone the pharmacy with the prescription number on the container. The pharmacy will either refill the medication or contact your doctor directly for an authorization to do the refill. It is not necessary to phone your physician for a refill. Do phone your physician if you have questions regarding how best to take your medicine or on side effects. Helpful tips:
Phone for refills before your supply runs out to avoid an emergency situation.
If you do need the medicine on the same day, be sure to tell the pharmacy so that they can try to accommodate your needs. Do not assume that they will do the refill on the same day.
BVMG recommends that you consult with your primary care physician (PCP) regarding referrals for specialty care. You may or may not need to first have an appointment with your PCP, depending on the medical circumstances and how familiar your PCP is with your medical condition. Many insurances require prior authorization for specialty care. Without such authorization, you may be responsible for the charges. Bay Valley Medical Group accepts most major health insurance plans including Medicare. Please refer to the Health Insurance Plans. If you have a question regarding a health condition of your own or a family member’s, please phone your physician. The staff will assist you in obtaining medical advice or in scheduling an appointment. For more general health information, Bay Valley Medical Group provides resources to patients and their families. When you have a question for your physician, telephone his or her office. A staff person will assist you and/or relay your message to your physician. Patient calls are returned throughout the day, in between office appointments, with a goal of returning morning calls before 12:30 pm and afternoon calls before 5:30 pm. Be sure to convey an urgency of a situation if one exists.
BVMG clinic sites do not currently offer phone or email appointments. Affiliate physician office policies vary and you may consult with the specific physician or his/her staff. |
